Care managers can evaluate an older person’s situation, make recommendations, arrange and oversee appropriate services and keep family members informed.
Care Managers act as an advocate in medical settings – such as the doctor’s’s office and hospital.
A care manager will try to address any needs that will keep a person safe, healthy and happy in their environment. That can be in their home, in the hospital or in a senior community. The services of a care manager are often needed when your care needs increase, due to an ongoing illness, a fall or surgery.
